HOLYOKE, MA (February 5, 2026) – This month, on Groundhogs Day specifically, MissionCare at Holyoke marked five years as part of iCare Health Network and as an active specialized program and care center serving all of Massachusetts—a milestone that reflects both growth and clarity of purpose.
Over the past five years, MissionCare at Holyoke has developed expertise in caring for individuals whose clinical and psychosocial needs require a higher level of coordination and support. The center welcomes residents from across Massachusetts, including those living with complex medical conditions and behavioral health diagnoses, as well as individuals connected to state agencies. Many arrive after experiencing barriers to placement elsewhere. MissionCare’s approach is to assess each referral carefully and build the interdisciplinary supports necessary to provide safe, appropriate, and person-centered care.
At its core, the center remains rooted in the fundamentals of skilled nursing. Whether supporting someone’s recovery following hospitalization or providing long-term care for medically fragile residents, the team focuses on dignity, stability, and individualized attention. It is this combination—enhanced clinical capability paired with consistent, compassionate daily care—that continues to shape MissionCare at Holyoke’s identity.
Since inception of the program the team has strengthened interdisciplinary care coordination, behavioral health supports, and clinical programming designed to meet residents where they are. This includes collaboration with state agencies, hospitals, and referral partners to ensure safe transitions and continuity of care for individuals who might otherwise struggle to find placement.
This milestone highlights the dedication of nurses, certified nursing assistants, therapists, social workers, and support staff who show up each day prepared to care for a population that requires patience, expertise, and resilience. And we cannot forget the dedication and skill of the department heads and leadership team who provide key guidance and focus on the mission.
Congratulations to the MissionCare at Holyoke team and a big thank you to all of our state agency and hospital partners across Massachusetts.
