Our mission is to provide a safe, secure environment that is inclusive, compassionate and free of stigma.
At 60 West, we are committed to actually living and delivering on our Mission and Vision. Our team is diverse, passionate and engaged, but most importantly they are a Team and a Family. We work together day in and day out to make 60 West all that it can be and bring that same feeling to the residents with whose care and safety we are entrusted.

We are pleased and proud that since inception, 60 West has consistently been rated a Four or Five Star facility overall and for quality measures, as granted by the Centers for Medicare and Medicaid Services (CMS). 60 West has also been the recipient of a US News and World Report Best Nursing Home recognition for 2018-2019 and 2019-2020 and an American Health Care Association (AHCA) Silver Quality Award. Happy Long Term Care Administrator's Week 2024

March 11-15, 2024 is National Long Term-Care Administrator’s Week. During this week, staff, residents, families, and volunteers have the opportunity to say “thank you” and honor the administrators who lead our nation’s long-term care communities. A heartfelt and sincere thank you from all of us at iCare Health Network to our dedicated Administrator team who continues to lead through challenging times and work tirelessly to safeguard and enrich the lives of the residents we serve. -
On February 28, 2024 the team at Touchpoints at Chestnut took a moment to celebrate a big achievement: the care center’s return to Five Star designation with the Centers for Medicare and Medicaid Services (CMS)! According to the CMS website, CMS created the Five-Star Quality Rating System to help consumers, their families, and caregivers compare nursing homes more easily and to help identify areas about which they want to ask questions. On February 6, 2024, as part of Black History Month, the team at Touchpoints at Chestnut celebrated the life of music legend Bob Marley. Staff and residents enjoyed listening to his music and shared Jamaican beef patties and cocoa bread. February 6th is Bob Marley Day. Touchpoints at Manchester Goes Red for American Heart Month
February 2, 2024
February is designated as American Heart Month to bring attention and awareness to the prevalence and severity of heart disease in the United States. Touchpoints at Manchester staff and residents wore red on February 2, 2024 for National Wear Red Day which is held the first Friday of every February to help raise awareness around heart disease. -
