BLOOMFIELD, CT (September 18, 2025) – Touchpoints at Bloomfield was proud to be a featured participant at the 18th Annual Bloomfield Senior Expo, held on Tuesday, September 16. The Expo brought together local seniors, caregivers, community partners, and service providers for a day of education, resources, and connection.
Touchpoints at Bloomfield was represented by Administrator Heather Rodriguez, Complex Care Nurse Mirja Tupas, Regional Clinical Director Francis Lijadu and iCare Nurse Liaison Jen Joffray. The team welcomed attendees to the Touchpoints booth and shared information about the wide range of clinical programs and services available at the care center.
Highlights included the center’s Bronze Quality Award from the American Health Care Association as well as its certified and renowned Heart Failure Program, which provides evidence-based care to reduce hospital readmissions, improve patient outcomes, and support residents in achieving their highest level of independence. Touchpoints at Bloomfield is proud to have earned American Heart Association certification in heart failure management—an achievement that reflects the dedication of staff and the quality of care provided.
In addition, Heather, Mirja, and Jen spoke with attendees about the many other clinical specialties and rehabilitation programs available at Touchpoints at Bloomfield. These include skilled nursing and long-term care services, complex medical management, short-term rehabilitation, and coordinated care pathways designed to help residents transition successfully from hospital to home.
