Huntington’s Disease Care at Fresh River
Huntington’s disease brings a unique set of challenges—physical, cognitive, and emotional—that progress over time. Placement options in Connecticut have been limited for years. Fresh River Healthcare in East Windsor is currently the only skilled nursing center in the state with a dedicated Huntington’s Disease Program.
We work closely with the UConn Health Huntington’s Disease Program. Their specialists help guide care, and a UConn Health APRN regularly assesses residents on site. Staff receive targeted training so they understand the specific needs that come with HD. Recent state grant funding allowed us to add specialized equipment—therapeutic seating, adaptive dining tools, floor-level safety beds, and mobility supports—that make daily life safer and more comfortable. The focus is always on helping residents maintain as much function and quality of life as possible, whether they are with us short-term or for longer stays.
HIV Specialty Care at Trinity Hill
More than half of people living with HIV in the United States are now over 50. That shift has changed what good care looks like. Many residents also face social isolation, mental health concerns, or other medical issues that need coordinated attention.
Trinity Hill Care Center in Hartford operates one of only two state waiver HIV/AIDS specialty programs in Connecticut nursing facilities. The dedicated unit includes weekly rounds with an infectious disease physician, careful management of antiviral medications, mental health support, and strong social work involvement to help with housing, community resources, and transitions. We also integrate services like wound care, therapy, and our iRecovery substance use program when needed. Partnerships with local organizations help fill gaps that a nursing center alone cannot address. The approach is practical and personal—focused on medical stability, reducing stigma, and supporting each resident’s overall well-being.
Tracheostomy Care and Other Complex Needs
Many of our centers are equipped to care for residents with tracheostomies. Having this capability means residents can often move out of the hospital sooner and receive ongoing support in a skilled nursing environment rather than remaining in acute care longer than necessary.
Beyond trach care, our network offers additional specialized supports: on-site dialysis at select centers, advanced heart failure and pulmonary programs, secured behavioral health units, the iRecovery program with medication-assisted treatment, care for acquired brain injury, and services for veterans. MissionCare centers focus on individuals who have been difficult to place elsewhere, including those with justice involvement or complex behavioral and medical needs.
What Ties It Together
These programs work because of the people behind them. Interdisciplinary teams—nurses, therapists, social workers, physicians, and support staff—collaborate closely. We maintain relationships with hospitals, specialty clinics, and community providers so care stays coordinated. Staff education is ongoing. Equipment and environments are adapted when needed. And we keep coming back to the same principles: education, nurturance, respect, innovation, commitment, and health.
