Touchpoints at Bloomfield Earns Heart Failure Certification from American Heart Association, Opens Heart Failure Suites

BLOOMFIELD (June 27, 2025) – Touchpoints at Bloomfield has earned the Skilled Nursing Facility Heart Failure Certification offered by the American Heart Association®, the world’s leading voluntary organization focused on heart and brain health.

The iCare and Touchpoints teams gathered at the care center on June 26th to celebrate this achievement, unveil the new certification signage and cut the ribbon on two new Heart Failure Care Suites. These suites are private rooms tailored to the clinical and personal needs of individuals receiving heart failure care in the post-acute setting. These special care areas and the work and dedication in obtaining the certification set Touchpoints Rehab out front as the leader in heart failure care in the skilled nursing center.

What does AHA Certification Mean - For Providers and Patients

The culmination of several years of preparation, construction of the program, partnerships and hard work, this certification is a testament to the commitment of Touchpoints at Bloomfield, iCare Health Network and its Complex Care Team. As such iCare would like to extend its thanks and congratulations to Mirja Tupas, Francis Lijadu, Heather Rodriguez and the entire team at Touchpoints at Bloomfield who joined in the celebration. 

For residents of Touchpoints at Bloomfield, this certification means heart healthy care standards, specialized staff training, better outcomes, quality measures aligned with the AHA, improved resident safety and coordinated care. Plus the trusted endorsement of the American Heart Association.

The certification provides an evidence-based framework for evaluating skilled nursing facilities against the American Heart Association’s rigorous science-based requirements for heart failure (HF) patients, including program management, patient and caregiver education and support, care coordination, clinical management and quality improvement.
