Pre-booking your post-surgery rehab will ensure you will receive the services of your choice. Planning your post surgery care ahead of time will reduce your stress and let you focus on recovery. You can pre-book for every level of care. Transition from hospital to post-acute inpatient rehabilitation; directly from hospital to home care; or directly to Outpatient Rehab.
We go “BEYOND THE JOINT” with our Touchpoints Prehabilitation; a unique and signature program. Our Prehab is designed for all Joint Replacement and Spinal Surgery patients. Our physical Therapist will look at your entire body and assess your individual needs so you will be in the best possible condition to receive your new joint or corrective surgery.

Touchpoints at Bloomfield Hosts Thanksgiving Luncheon 2023 at Bloomfield Senior Center
November 17, 2023On November 16, 2023 the team at Touchpoints at Bloomfield once again hosted a Thanksgiving luncheon at the Bloomfield Senior Center. About 75 Bloomfield seniors were in attendance as several department managers from Touchpoints at Bloomfield served a traditional holiday meal complete with appetizers, dessert and apple cider. MissionCare at Holyoke 'Lights the World in Teal' to Raise Awareness and Show Support for Persons with Alzheimer's Disease
November 8, 2023The team at MissionCare at Holyoke ‘went teal’ this week to show support for and raise awareness of individuals living with Alzheimer’s disease, especially the residents of the care center. Light the World in Teal is an annual program held in November as part of Alzheimer’s Awareness Month. People and places around the world “go teal” to raise awareness and show support for the millions of people affected by Alzheimer’s.
On October 31, 2023, the team at Parkville Care Center celebrated and recognized Maria Parks for her 37 years of service to the residents of Parkville, stretching back to when it was Park Place and before that, Greenwood Manor. Maria is an LPN Charge Nurse who is beloved by staff and residents and was a mentor to new nurses. She was present for the Greenwood fire in 2003 and spoke this year at the 20th anniversary remembrance ceremony. Maria’s retirement party included many emotional …

On Saturday, October 28, 2023 a team from Parkville Care Center took part in the 2023 Walk to End Alzheimer’s - Greater Hartford and raised over $1000 for advancing the fight against Alzheimer’s Disease. The walk took place at Rentschler Field and according to the Alzheimer’s Association website in total had over 1,300 participants in 275 teams and raised $300,000 towards their goal. iCare Health Network Celebrates Greater Hartford Puerto Rican Day 2023, Marches in Parade
October 9, 2023On October 8, 2023, staff and their families from iCare’s Parkville Care Center and Trinity Hill Care Center represented iCare Health Network in celebrating the 2023 Greater Hartford Puerto Rican Day Parade and Festival del Coqui. As they did in 2019 and again in 2022, the iCare team marched in the parade from Wawarme Avenue, ending in Bushnell Park, decked out in matching t-shirts and regalia and accompanied by decorated Jeeps.
