Pre-booking your post-surgery rehab will ensure you will receive the services of your choice. Planning your post surgery care ahead of time will reduce your stress and let you focus on recovery. You can pre-book for every level of care. Transition from hospital to post-acute inpatient rehabilitation; directly from hospital to home care; or directly to Outpatient Rehab.

We go “BEYOND THE JOINT” with our Touchpoints Prehabilitation; a unique and signature program. Our Prehab is designed for all Joint Replacement and Spinal Surgery patients. Our physical Therapist will look at your entire body and assess your individual needs so you will be in the best possible condition to receive your new joint or corrective surgery.

Available for patients preparing for total hip, knee, ankle, shoulder replacements and spinal surgery. Our patients rate us in the top percentiles for overall satisfaction with care and results. Your personal program navigator will support you from the first phone call until discharge from one of our care centers.

  • Every year on September 18th, National HIV/AIDS and Aging Awareness Day shines a light on the unique challenges faced by older adults living with HIV. With improved treatment and earlier diagnoses, many individuals are living longer, healthier lives. Today, more than half of people living with HIV in the United States are over the age of 50. This makes it more important than ever to focus on specialized care, stigma reduction, and supportive services that meet the needs of an aging population.
  • Parkville Care Center proudly joined the Greater Hartford community on Sunday, marching in the 2025 CICD Puerto Rican Day Parade. The annual event, organized by the Connecticut Institute for Community Development (CICD), is a vibrant celebration of Puerto Rican culture and heritage, drawing thousands of participants and spectators to Hartford’s streets. Parkville Care Center and iCare Health Network have been proud supporters of CICD have marched in the parade for several years. 
  • Touchpoints at Bloomfield was proud to be a featured participant at the 18th Annual Bloomfield Senior Expo, held on Tuesday, September 16. The Expo brought together local seniors, caregivers, community partners, and service providers for a day of education, resources, and connection.   • On August 26th, twenty-one leaders from across iCare Health Network nursing facilities – including Administrators, Program Directors, and other care team members – successfully completed the Behavioral Health Ambassador Training through the Center of Excellence for Behavioral Health in Skilled Nursing Facilities (COE-NF). Each participant earned two professional credentials: the COE-NF Behavioral Health Ambassador Badge and CPI Verbal De-escalation Strategies Certificate.
